Insurance & Billing

Collaborative Dental Care is a participating provider of Delta Dental’s PPO and TRICARE Retiree Dental Program.  If you have a different insurance, we are able to bill that insurance directly for your reimbursement.  Please feel free to call us for details or contact your network.  

Payment Options

We accept checks, cash or credit cards.  If you are a conservator for a patient or have Power of Attorney, it is important we have your complete contact information as the person responsible for payment prior to any appointment.
